Texas Parent to Parent provides a variety of resources, education and training to parents, family members, professionals, and others who share an interest in improving the transition opportunities for youth and young adults with a disability, chronic illness, or special health care needs. Read on to find out more about how you can join our family and connect with other parents.

Pathway to Adulthood

When children with disabilities, chronic illnesses, or other special health care needs approach adulthood, parents face many issues in assisting their young adult start their life after school. Just as milestones bring new worries and concerns, transition is the most worrisome because there are few resources or agencies to help. Pathways to Adulthood is designed to educate and empower parents so that they know what options are available and help them make plans for this important time.

Fortunately, many parents have “gone before” and can be a source of strength, hope, direction and help. Peer support is the heart of Texas Parent to Parent’s (TxP2P) service to parents — a parent never need feel alone or isolated. A parent match specifically connects the requesting parent with an experienced supporting parent who can:

  • help with understanding how things change for your child at age 18 and at 21
  • help with transition resources,
  • listen to you and help you & your child make a plan for the future, and
  • offer emotional support.

Parent-to-parent matching is available to any parent who requests it. If you would like to request a “Parent Match” for transition issues, complete the following form so we can better assist you. In addition, consider attending the next TxP2P Parent Conference which has numerous sessions on this topic and a Teen Transition Summit designed specifically for young people who are approaching adulthood. TxP2P also has a listserv that discusses topics related to transition that you may join.
